In compliance with OM-OUOPS-2024-09-10330 regarding the request to conduct Midline Nutritional Assessment among School-Based Feeding Program (SBFP) beneficiaries, all SBFP implementing schools shall submit Midline Nutritional Assessment Form using the attached form as Annex A.
