In compliance with DM No. 114 s. 2017 entitled ‘Reiteration of the Reportorial

Obligation of Schools, Schools Division Of‌lices, and Regional Offices Pursuant to

DepEd Order Nos. 40, s. 2012, 55, s. 2013, and 18, s. 2015,” and the attached

Memorandum from the Office of DepEd Undersecretary for Operations Revsee V.

Escobedo dated June 8, 2023 with subject “Request on the Consolidated Reports

on Learner Rights and Protection Cases” all schools are directed to accomplish the

following school reports, which will be consolidated by the district school heads

in-charge of learner rights and protection:
