Requirements
Education: Bachelor's Degree in Elementary Education; or Bachelor's Degree with 18 Professional Education Units
Experience: HT for 1 year; or TIC for 2 years; or MT for 5 years
Training: 40 Hours relevant training
Eligibility: RA 1060 (Teacher)
Documents Needed
Fully accomplished PErsonal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212 Revised 2017) which can be downloaded at www.csc.gov.ph;
Official Transcript of records (Bachelor's degree / Masteral/ Doctoral degree / units earned)
Photocopy of certificate of eligibility/ rating/ license; and
Performance Ratings for the last 3 rating periods (if applicable)
Updated Service Records (if applicable);
Lates appointment papers (if applicable); and
Certificate of trainings of atleast 3 days (if applicable).
