Education: Academic Track: Bachelor's degree with a major in the relevant strand/subject; or any Bachelor's degree plus at least 6 units towards a Master's degree in relevant strand/subject.
TVL Track: Bachelor's Degree; or completion of the technical-vocational course(s) in the area of specialization
Sports Track: Bachelor's degree with a major in the field(s) under the Track; or any Bachelor's degree plus 15 units of specialization in fields under the Track
Art & Design Track: Bachelor's degree with a major in field(s) under the Track; or any Bachelor's degree plus 15 units of specialization in the relevant subject

1.Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212 Revised 2017) which can be downloaded at www.csc.gov.ph;
2.Official Transcript of records (Bachelor's degree / Masteral/ Doctoral degree / units earned)
3.Photocopy of certificate of eligibility/ rating/ license; and
4.Performance Ratings for the last 3 rating periods (if applicable)
5.Updated Service Records (if applicable);
6.Lates appointment papers (if applicable); and
7.Certificate of trainings of at least 3 days (if applicable).
