Requirements
Education: Bachelor's Degree in Elementary Education; or Bachelor's Degree with 18 Professional Education Units
Experience: HT for 2 years; or TIC for 2 years; or Teacher for 5 years
Training: 24 Hours relevant training
Eligibility: LET, RA 1060 (Teacher)
Documents Needed
1.Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212 Revised 2017) which can be downloaded at www.csc.gov.ph;
2.Official Transcript of records (Bachelor's degree / Masteral/ Doctoral degree / units earned)
3.Photocopy of certificate of eligibility/ rating/ license; and
4.Performance Ratings for the last 3 rating periods (if applicable)
5.Updated Service Records (if applicable);
6.LatesT appointment papers (if applicable); and
7. Certificate of trainings of atl east 3 days (if applicable).
